INTAKE PROCEDURE: Call to schedule an appointment to establish care. Same day appointments may be available for existing patients who have already established care.
FEES: Accepts OHP, Medicare, and most private insurances. Sliding scale fees available. No one turned away because of inability to pay for services.
DOCUMENTS: Insurance information, if applicable. Income information if applying for sliding fee scale. A list of medications you are taking and questions for your providers.
