MATERNAL CHILD HEALTH NAVIGATION
Provides access to resources for families in the perinatal period through the first 6 years of the child's life. Helps pregnant people get enrolled with OHP, including Oregon Mother's Care. Connects families with developmental screenings, benefits navigation, and provides individual and family support.

Maternal Child Health Navigation is part of Clackamas County Access to Preventive Health (APH). APH centers access and reduces barriers to care across services to improve immunization rates, reproductive health, and maternal and childhood health and nutrition.
