Eligibility:
Households must:
-Be very low-income
-Be chronically homeless. "Chronically homeless" under this program means that the applicant is currently living on the street or other place not meant for human habitation (parks, vacant buildings, etc.) and has been homeless continuously for one year or has had four episodes of homelessness in the last three years. It does not mean "at risk of homelessness sometime in the future," or staying with family/friends.
-Have at least one adult household member with a disability. A person is considered to have a disability if the person has a physical, mental, or emotional impairment which is expected to be of long-continued and indefinite duration, substantially impedes the person's ability to live independently, and could be improved by more suitable housing conditions.

INTAKE PROCEDURE:
Must be referred by a social service provider when funds are
announced and complete an application during the application
period. Does not maintain a waitlist.
