INTAKE PROCEDURE:
Paternity Testing: When calling to make appointment, you must provide following information: mother's and alleged father's names, child's name and date of birth, the name of the person responsible for payment and the telephone number of the scheduling client. Buccal swab sampling and blood sampling available.
