CASE MANAGEMENT
Uses the Strengths Model and the Assertive Community Team Model for psychiatric case management. Case managers help people recognize and utilize their individual strengths, abilities, and potential to secure community resources that will help them attain life goals in the areas of employment, education, housing, social support, and medical services. Case managers proactively coordinate services to increase client self-reliance, promote community activity, and decrease unnecessary and prolonged reliance on mental health services.
