Eligibility:
Individuals, youth 18-24, and families experiencing homelessness
will receive a Coordinated Entry System Housing Needs Assessment if
they are:
-Living and sleeping outside, in a car, or in places not meant for human habitation
-Staying in a shelter or transitional housing
-Fleeing/attempting to flee domestic violence
-Exiting an institution (jail, prison, a psychiatric hospital, medical hospital, or a drug/alcohol treatment program) where they resided for up to 90 days and were homeless before entering that institution

INTAKE PROCEDURE:
Call during service hours to complete a housing needs assessment.
After the assessment, a CES advocate will notify clients if/when a
housing program opportunity is available. There are no guaranteed
timelines for being notified. Housing referrals are determined by
funding availability, are limited, and not regularly available.
