Mental health consultation, evaluation, treatment, and case management for seniors in care facilities and referred from private medical practitioners. Enhanced care outreach services include intensive case management, counseling, and behavioral interventions for senior and physically disabled adults with serious and persistent mental illness.

Hours: Monday-Friday 8am-5pm
