Provides an online locator tool and phone line for finding approved physicians (civil surgeons) to conduct immigration medical exams. Medical examinations are typically required for adjustment of status cases (green card). The doctor will test for communicable diseases, such as tuberculosis and syphilis, and will also check your records to see if you need any vaccinations.

The exam must be done by a doctor who is authorized by US Citizenship and Immigration Services (USCIS). Military physicians are authorized to perform immigration medical exams at a military treatment facility within the US for veterans, members of the military, and designated dependents.

It may be helpful to call a few civil surgeons to find out how much they charge for the immigration medical examination. Prices can vary by several hundred dollars.

DOCUMENTS: Bring the following to your medical exam:
-Vaccination or immunization record
-Health insurance card, if applicable
-Form I-693, Report of Medical Examination and Vaccination Record. Print out and bring the most recent version. Fill out Part 1, but do not sign until the civil surgeon instructs you to do so. The civil surgeon will complete, sign, and seal the form and any supporting documents in an envelope.
-Government-issued photo ID (i.e., valid passport or driver's license). If you are 14 years old or younger, bring ID that shows your name, date and place of birth, and parent's full name. Possible forms of ID include a birth certificate (with an English translation) or an affidavit.
-Payment; check with medical office about acceptable forms of payment
