INTAKE PROCEDURE: Call for appointment. Prospective clients are interviewed by phone and then an appointment is arranged for an intake assessment.
FEES: Sliding scale fee; accepts Oregon Health Plans (OHP) are Oregon, Health Share, Columbia Pacific, and Trillium. Can not bill to private insurance plans or Medicare.
