CACOON
CaCoon is a statewide public health nurse home visiting program that provides community-based CAre COordinatiON for children with special health needs. Children served in the CaCoon program have higher rates of immunization, annual well-child visits, and dental care visits; and make fewer visits to the emergency department. Since family members have a central role in the care of their child, all services are planned around the desires and concerns of the family.
