NEIGHBORHOOD HOUSING AND CARE PROGRAM
Offers in-home support services including nursing, occupational therapy, and social work. Services may include health assessment, education and management of medication, coordination of assistance to perform self and household care, coordination of housing support (rental assistance to those who have no other means of paying for housing), fostering engagement and connection in the community, care coordination and advocacy, counseling and social support, home safety assessments, connection to community resources (for material, emotional, health, vocational, and leisure needs), and education/consultation and practice of life and coping skills.
