INTAKE PROCEDURE: Call for information and to schedule an appointment. Walk-ins accepted for crisis services.
FEES: OHP, Medicare, private insurance, sliding scale, payments
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
