INTAKE PROCEDURE: Call for initial intake, which is a 15-20 minute interview. Waitlist is currently 8-10 weeks to be scheduled with a counselor.
FEES: Accepts most private insurance (no OHP (Medicaid) or Medicare). Sliding scale fee available is for uninsured patients.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
