Community health center providing comprehensive primary health care, covering neonatal, pediatric, adult, and geriatric services. Specific services include prenatal care, preventive services, immunizations, EKGs and other diagnostics, minor surgery, diabetes screenings, gynecological services, contraception, laboratory services, prescriptions and medication assistance, dental screenings, mental health evaluations, screenings for sexually transmitted infections, and referrals to specialty treatment. Provides urgent care clinic for current patients only.

In addition, the Homeless Health Care Clinic provides acute primary care for homeless individuals.

INTAKE PROCEDURE: Call for intake appointment to be admitted as a new patient. After the intake appointment, an appointment with a doctor will be arranged.
FEES: Accepts Oregon Health Plan (Medicaid), Medicare, Tricare, most private insurance. Offers a sliding fee scale based on income and family size, for those without insurance and those with minimal insurance coverage. No one is turned away for an inability to pay.
