PATIENT CARE PROGRAM
Helps finance eye and ear surgeries for low income individuals who cannot afford treatment. The following procedures may qualify for assistance:

-Vision: cataract surgery, corneal transplant, enucleation, retinal injection, autologous serum eye drops, dry eye treatment, medical contact lenses, corneal crosslinking, insurance co-pays and deductibles, prosthetic eye, pterygium surgery, and more.

-Hearing: adenoidectomy, tympanoplasty, mastoidectomy, myringotomy, expenses related to implantable hearing devices, and more.

DOCUMENTS: After referral is received, applicant will need to complete a 3-page application, include recent acceptable forms of proof of household income, and include chart notes from recent eye or hearing exams.
LANGUAGES: English, interpretation by arrangement. Applications available in English and Spanish. Website translated to Chinese, English, French, Korean, Romanian, Spanish, and Vietnamese.

TRAVEL/LOCATION INFORMATION: If visiting in person, please use the Legacy Devers Eye Institute door located at 1040 NW 22nd Ave. Go past the Devers reception area. Turn right to go down the hallway. Go past the elevators to the end of the hallway. Our office is at the very end of the hallway and is in the same suite/office as Eye Promise Optical.
