Offers a range of support services to individuals experiencing intellectual and/or developmental disabilities. Services can include case management, individual and family support plans, access to brokerage services for transportation, in-home supports or residential placement, and employment and vocational services.

INTAKE PROCEDURE: Call to request an application for Developmental Disabilities services. An appointment will be scheduled with an eligibility worker who can help you through the application process.
FEES: None
DOCUMENTS: Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
