Assists families of children and youth with special health care (CYSHCN) needs and the professionals who serve them identify sources for health care funding. Provides advice, information and referral, and taps into a network of other families and professionals for support and information.

Eligibility: Families with children and youth with special health care needs ages 0-26. CYSHCN are children and youth who have or are at risk for developing chronic physical, developmental, behavioral, or emotional conditions and who also require health and related services of a type or amount beyond what is generally required by children and youth.
