-Age 60 and older or adults under age 60 with a diagnosis of
Alzheimer's-type dementia or similar similar cognitive
impairment
-Ages 19-59 with a physical disability
-Eligibility requirements involve an in-home assessment to determine activities of daily living and a financial determination to establish the fee for service
