Provides primary medical care for adults and children, as well as immunizations, mental health services (evaluations and counseling), women's reproductive health (Pap tests, breast exams), family planning and contraception, pregnancy testing, prenatal services, and drug and alcohol assessments.

Also assists in applying for the Oregon Health Plan and private insurance plans on the Federal Health Exchange.

INTAKE PROCEDURE: Call to schedule an appointment
FEES: Accepts OHP, Medicare and private insurances. $35 office visit fee applies for uninsured patients and sliding scale fees are available to cover the remaining costs. No one will be turned away for inability to pay.
DOCUMENTS: Insurance information, if applicable. Proof of income if applying for sliding scale fees.
