Eligibility:
Must be currently enrolled in Oregon Health Plan, Medicaid
eligible, or have received service in the past 12 months AND
qualify under one listed condition.
-Age 65 and older
-Diagnosed with a medical condition that makes the applicant especially vulnerable to extreme heat, such as Respiratory disease (asthma or chronic obstructive pulmonary disease), Heart disease, Diabetes, Hypertension (High blood pressure), or have children who are medically fragile living in the home
-High-risk: Applicant speakes a language other than English, race is other than “white,” pregnant, and has children under the age of 6
-Age 65 and older
