211info is the main agency that is placing families into the Coordinated Entry for Families MHTQ system. Please do not call or walk in to any of the partner agencies, as they will refer back to 211info.
Coordinated Entry for Families/MHT is not an immediate or guaranteed resource and is based on availability of services and individual eligibility.
Eligibility:
Families (any number of adults) with children in their custody
under the age of 18 (or plans for reunification with children under
18) - OR - pregnant women in their third trimester of pregnancy (28
weeks/7 months) who are currently experiencing homelessness or are
at-risk of homelessness AND are reside in or have ties (work,
school) to Multnomah County AND meet one of the following
criteria:
-Staying in a place not meant for human habitation (e.g. vehicle, abandoned building, anywhere outside)
-Doubled up, couch surfing/staying with family or friends, motel, shelter, institution AND must vacate within 14 days
-Housed with a FOR CAUSE EVICTION due to violation of rental agreement AND a court ordered date to vacate property within 14 days
-Actively fleeing Domestic Violence without a safe place to go
Please note: This program is not an immediate resource and cannot assist financially with evictions to maintain current housing. MHTQ may be able to help with future housing if the tenants end up vacating housing due to the eviction. For eviction prevention seek rental or eviction prevention assistance.

Hours:
24 hours per day / 7 days per week to be added to callback list to
complete screening intake assessment
8am-6pm 7 days per week for screening intake assessment
8am-6pm 7 days per week for screening intake assessment
