Connects community members to the Coordinated Entry Systems for housing in Multnomah, Washington, Clackamas, and Clark Counties. Housing/rental assistance is reserved only for households referred to us by the Coordinated Entry Systems. Staff work with each household to develop an individual support plan to identify goals and strategies designed to help achieve housing stability.

Health-related services (HRS) are non-covered services that are offered in addition to covered benefits of the Oregon Health Plan (OHP) through Coordinated Care Organizations (CCOs) to improve care delivery and overall member and community health and well-being. HRS may also be referred to as flexible services, or flex/flexible funds.

HRS are NOT intended to address emergency situations. CCOs are not required to offer flexible services. A CCO can decide to approve or deny flexible services requests on a case-by-case basis. Each CCO sets its own standards for which services to approve.

Individuals who are dual enrolled in both Medicare/Medicaid might be eligible for air conditioners.

HRS vary by an individual's CCO, and may include:

-Helping a person get a cell phone if having one will give them better access to their providers for telehealth
-Transit passes for members who need transportation for health-related needs beyond covered appointments
-Providing an air conditioner for a person whose medical diagnosis is affected by heat
-Accesses to a yoga studio or gym membership for a person whose back pain would be improved by an exercise class
-Preventing utilities shutoff to maintain service to medical equipment
-A class on cooking healthy meals for a person with diabetes
- Limited assistance with finding housing, move-in costs, and maintaining housing for OHP members facing certain life challenges

Provides short-term, emergency eviction prevention assistance.

Provides basic financial assistance with initial move-in costs such as a security deposit, application fee, or cleaning fee for an apartment. Also offers case management support to youth ready to live on their own.

The Independent Living Program (ILP) helps youth who are, or were, in foster care to become self-sufficient adults. It gives an opportunity to learn valuable skills necessary to make a successful transition from state and Tribal custody to living on their own in the community. Services include access to case management and funds for continuing education and assistance with housing.

Polk County Resource Center provides the following services: Emergency financial assistance for rent, utilities, birth certificates, ID cards, and prescriptions; hygiene items, diapers, transportation passes, shower passes and gas cards; comprehensive information and referral; child care resources and referrals and education to providers and parents. Also offers seasonal programs including school supplies and holiday assistance.

May be able to assist with rent, deposits, utilities, job search costs, child care costs, or clothing or tools needed for work.

Provides assistance to those with housing needs, including payments for rent, rental deposit, and rental applications. Also provides screening for emergency shelter. Assistance is not guaranteed.

Provides limited financial assistance with emergency situations such as owed rent, electricity, gas, water, and small medical bills. May also provide assistance with rental deposits and minor car repairs pertaining to safety issues. Prospective clients must be able to demonstrate ability to sustain needs post assistance.

Persons who have never received aid from the agency before may be given priority in applications.

Tenant-Based Assistance (TBA) program may provide rental assistance to very low-income tenants by paying a portion of current or future rent, refundable security deposits, rental application fees, or potentially utility deposit fees. Unable to cover costs for past debts accrued - assistance is to maintain or secure current housing. Households are required to meet with a case manger on an ongoing basis and make progress in their goals and plans for self-sufficiency. Approved applicants are funded for an initial 6-12 month period, although assistance may be extended for up to 24 months.

Provides a short-term housing intervention program for low-income veterans and their families experiencing homelessness or at imminent risk of becoming homeless.

Veterans and their families can be assisted with housing through case management, pending eligibility. If eligible, assistance and support may include move-in/rental assistance, eviction prevention, benefits support, employment and education services, legal aid around housing barriers, and other expenses that may support stable housing.

Must be willing to work with a caseworker. May require clients to be alcohol- and drug-free. Assistance in any of these programs depends on availability of funds.
