Provides a continuum of care for people with mental health challenges, designed to help consumers move toward maximum recovery and successful community life. Services include individual and group therapy, skill development, targeted case management, hospital discharge services, dual diagnosis treatment, peer-delivered services, representative payee services, residential and 20-hour personal care services, and coordinated care services. Most services are provided at client's residence.

OREGON FAMILY TO FAMILY HEALTH INFORMATION CENTER
Oregon Family to Family Health Information Center (OR F2F HIC) operates a call-in center for family members of children and youth with special needs and chronic health conditions (developmental and emotional disabilities, behavioral problems, and chronic physical illnesses) and professionals who serve them. The center specializes in providing information and resources related to health information, health care access, and special needs systems and supports. All calls are confidential.

Families may also request to be placed on an email list to receive monthly correspondence about issues of importance to children and youth with special needs in Oregon.

Eligibility: Family members of children and/or youth (ages 26 and younger) with special physical, emotional, behavioral, or mental health needs, including those who are at risk or suspected of having a special need. The child/youth does not have to have a diagnosed condition.

Family to Family does not require documentation to prove citizenship to receive services.

YOUNG ADULT PROGRAM SENDER HOUSE
Provides transitional support for clients who struggle with mental illness. Program encourages independent living and helps clients manage the balance of everyday life in conjunction with self-care. Clients live at a group home managed by a Masters-level manager and skills trainers. Therapy is not provided onsite by staff; clients instead visit clinicians in the community and practice daily skills in a group setting. Clients use this program as a stepping stone to living on their own.
