HEALTH RELATED SOCIAL NEEDS HOUSING SUPPORTS
Health-related social needs (HRSN) are basic needs that affect a person's health. These benefits help members who qualify with basic needs including rent payment assistance and utility payment assistance; tenant support; storage fees; and home changes for health and safety including cleanup and modifications.

Eligibility: OHP members who meet all of the folllowing:
-Have a qualifying health condition
-Have a household income 30% or less of the average yearly income where you live
-Need support staying in your housing
-Have a lease or written rental agreement
-Lack resources to prevent homelessness
-Individuals who are age 65 or older or have children in the home who are age 0-5 and covered by OHP are also eligible
