DOCUMENTS: Income verification: Most recent W2 Form, Tax Return, State and/or Federal Low-Income Program Approval Letter, and 2 current months of bank statements required for all household members age 18 and older

One of the following:
Age verification: State issued ID, Real ID, Passport, Work VISA, Employment Authorization Card
Disability verification: Parking Permit ID, SS Disability Award Letter, Physician’s Certification Form
Military verification: Active Military ID, Discharge Paperwork, DD214
AREA SERVED: Cowlitz County
