Eligibility:
-Must be disconnected or have a pending disconnect
-Applicant must be listed on the account and reside at the service address
-Applicant must have active service for a minimum of 2 months
-Low income up to 200% of the Federal Poverty Level (income chart on apllication)
-May not exceed the higher of 80% of area median household income
-Not eligible if previous fraud events or on Moratorium Payment Plan
-Not available for deposits, collection balances, or fees

INTAKE PROCEDURE:
Visit website and complete the online Warm Neighbor Application.
Can also download the application online or pick one up at the main
office. Return the application along with income verification to
the office.

DOCUMENTS:
Income verification: Most recent W2 Form, Tax Return, State and/or
Federal Low-Income Program Approval Letter, and 2 current months of
bank statements required for all household members age 18 and older
