INTAKE PROCEDURE:
Call for an appointment when the line is open. Leave a message with
the applicant's name, phone number, Social Security number, and
utility company name to request an appointment. Please speak
clearly and slowly.

-Current utility bill in applicant's name
-Social Security card or copy of Social Security number cards for all household members
-Photo ID for all adults in the household
-Proof of income for the past 30 days for all adults in the household (TANF, SSI, unemployment, pay stubs, pension, child support, etc.)
