Eligibility:
Members of CareOregon who meet all of the following:
-Age 65 or older, children age 0-5, pregnant or gave birth in the past 12 months, OR anyone with a qualifying health condition
-Have a household income 30% or less of the average yearly income where you live (does not apply for home modifications)
-Need support staying in your housing
-Have a lease or written rental agreement
-Lack resources to prevent homelessness
