CLACKAMAS FREE CLINIC
Provides primary medical services for individuals without access to health care. Volunteer medical professionals offer routine preventative and non-emergency medical care. For chronic conditions such as hypertension, diabetes, asthma, and heart disease, care is provided in addition to education and equipment for self-care. Eye exams and glasses are also available. More specialized services that cannot be provided on-site are referred to a network of specialty physicians in the region.

LIONS SIGHT & HEARING FOUNDATION OF COWLITZ COUNTY
Provides eye and hearing screenings, eyeglasses, and hearing aids to those in need. Donations of used eyeglasses and hearing aids are accepted by the Northwest Lions Eyeglass Recycling Center, which are sorted, cleaned, and prepared for distribution within the community.

NEW EYES EYEGLASSES E-VOUCHER PROGRAM
Provides people in need with basic prescription eyeglasses through a nationwide e-voucher program. Please note that New Eyes does not provide or pay for eye exams.
Eligibility: This application is only for individuals who do not have a social worker, case manager or other health advocate. Serves all ages. Must have net monthly income at or below 250% of Federal Poverty Guideline, have an eyeglass prescription dated within the last 24 months, have no other resources available to pay for glasses, including federal/state programs or assistance from local charitable organizations, and have not received a New Eyes voucher in the past 12 months

Provides referrals to local Lions Clubs, which assist people in need of eye exams, eyeglasses, hearing exams, and hearing aids. Assistance depends on eligibility and funds available at each local club. No assistance with regular contact lenses for vision correction.

Provides limited emergency utility assistance (electric and water) and issues vouchers to St Vincent de Paul thrift store for free clothing, household items, and emergency furniture. Limited funding is available for eyeglasses, and bus passes for work or medical transportation only. Financial assistance is available once every 18 months.

Clients are asked to bring their own bags for food pantry and thrift store items.
