Eligibility:
Individuals and families who meet ALL of the following
requirements:
-Are a resident of Multnomah County
-Are at substantial risk of experiencing homelessness
-Are at or below 80% of area median income (AMI)

-Identification for all adults over the age of 18 such as a
driver's license, state ID card, Social Security card, passport, or
birth certificate
-Proof of income such as a pay stub or award letter for Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), child support, or TANF. Self attestation may also be accepted on a case by case basis.
-Copy of eviction notice or rental agreement
