Provides information and referrals to long-term appropriate community resources for more affordable and low-income housing options. No rental or deposit assistance.

-Help finding affordable housing and employment
-Access to additional school and community based resources

Housing units (a mix of single family homes and multi-family housing located in Bend, Oregon) are prioritized for homeless, low-income veterans. COVO also assists homeless, low-income veterans with housing search for rentals throughout Central, Oregon.

Youth shelter providing temporary emergency shelter for up to 120 days with written parent/guardian consent or 72 hours without written consent. Services for sheltered and unsheltered youth include case management, mentoring, housing search assistance, and counseling services. Bathing facilities, laundry facilities, a clothing closet, and personal grooming supplies are also provided. School supplies are for enrolled participants only.

Health-related services (HRS) are non-covered services that are offered in addition to covered benefits of the Oregon Health Plan (OHP) through Coordinated Care Organizations (CCOs) to improve care delivery and overall member and community health and well-being. HRS may also be referred to as flexible services, or flex/flexible funds.

HRS are NOT intended to address emergency situations. CCOs are not required to offer flexible services. A CCO can decide to approve or deny flexible services requests on a case-by-case basis. Each CCO sets its own standards for which services to approve.

Individuals who are dual enrolled in both Medicare/Medicaid might be eligible for air conditioners.

HRS vary by an individual's CCO, and may include:

-Helping a person get a cell phone if having one will give them better access to their providers for telehealth
-Transit passes for members who need transportation for health-related needs beyond covered appointments
-Providing an air conditioner for a person whose medical diagnosis is affected by heat
-Accesses to a yoga studio or gym membership for a person whose back pain would be improved by an exercise class
-Preventing utilities shutoff to maintain service to medical equipment
-A class on cooking healthy meals for a person with diabetes
- Limited assistance with finding housing, move-in costs, and maintaining housing for OHP members facing certain life challenges

Intensive case management for houseless teen birthing parent. Case managers assist in obtaining services for pregnant youth such as parenting classes, housing assistance, life skills training, health care, childcare, employment, or staying in school.

Provides basic financial assistance with initial move-in costs such as a security deposit, application fee, or cleaning fee for an apartment. Also offers case management support to youth ready to live on their own.

Homeless drop-in center for youth providing laundry facilities, phone use, temporary mailing address, and showers. Provides meals every weekday. Also provides street outreach, case management, mental health counseling, education, and assistance finding housing. Eligible youth will be added to a central housing referral list for all programs they qualify for.

The Independent Living Program (ILP) helps youth who are, or were, in foster care to become self-sufficient adults. It gives an opportunity to learn valuable skills necessary to make a successful transition from state and Tribal custody to living on their own in the community. Services include access to case management and funds for continuing education and assistance with housing.

Provides assistance in transitioning out of homelessness, transitioning out of sheltered settings, support for recovery based housing options, and information and referral to assist in overcoming barriers to permanent housing.

These grants do not provide immediate assistance, are sometimes one-time, and may include case management. Help with finding market rate or subsidized housing or linkage to community housing programs. Assistance with expediting VA Benefit claims and access to employment programs such as Supported Employment, Veterans Reintegration program, and State employment. Access to computers, clothing, and hygiene items is available onsite.

Services may include health screening, medical and dental care, vision, housing resources, counseling and substance abuse referrals, haircuts, pet care, job fairs, help accessing VA services and benefits, DMV and legal assistance. Meals, clothing, and hygiene supplies may also be offered.
