OREGON VITAL RECORDS
Maintains and issues all vital records for the State of Oregon, including birth, death, marriage, domestic partnerships and divorce records. The office has birth and death records dating from July 1903, marriage records from 1906, and divorce certificates (not decrees) from 1925.

For birth and death records within the first 6 months of issuance, copies are also available at the local County Health Department. Domestic Partnership Declaration forms can be obtained by mail through the Center for Health Statistics or from individual County Clerk's offices.

Eligibility: Birth, marriage, divorce, and domestic partnership records provided to immediate family and legal representatives. Death records provided to family members and persons with a personal or property right.

Vital records may be ordered online, by mail, by phone or by walking in to any Vital Records office.
FEES: -Online orders: $43.25 certified copy, $25 for each additional copy
-Phone orders: $44.95 for one record, $25 for each additional copy
-Mail orders: $25 for one record (allow 8-10 weeks for processing), $25 for each additional copy
-Rush mail orders (send order form by overnight delivery and pay with a money order): $25 for one record, $25 for each additional copy
-Walk-in orders: $28 for one record, $25 for each additional copy
DOCUMENTS: Social security number and valid ID. Call about other documentation that may be necessary.
