OREGON IMMUNIZATION PROGRAM
The Oregon Immunization Program provides technical assistance to vaccine providers and offers education and quality improvement for providers. Promotes immunizations for all age groups. Provides information and answers questions about immunizations for the public, as well as school requirements and the vaccine exemption process.

Each year the program reminds parents of the state law that requires that all children in public and private schools, preschools, Head Start and certified child care facilities have up-to-date immunizations, or have a religious exemption. Parents or guardians receive a letter in late January or early February if their child's immunization record shows that the child has not received the required vaccinations. Children will not be able to attend school or child care starting on a certain date in February, Exclusion Day, if their records on file show missing immunizations. Parents must make sure that their child's vaccinations are brought up to date and submit updated documentation to the school.

Oregon allows medical and religious exemptions to the vaccination requirements. For medical exemption, see doctor or county health department. For religious exemption, parent must sign religious exemption section of Certificate of Immunization Status, as well as see a doctor about or watch videos on the benefits of vaccinations.
