DEATH WITH DIGNITY ACT

A permissive law that allows terminally ill adults to end their lives through the voluntary self-administration of a lethal medication, expressly prescribed by a physician for that purpose. The website offers information on frequently asked questions, forms for patients and providers, and other end-of-life resources.

Patients who meet the criteria can request a prescription from a licensed Oregon physician. The physician must be a Doctor of Medicine or Doctor of Osteopathic Medicine licensed to practice medicine by the Oregon Medical Board.

Eligibility: -Age 18 years and older
-Capable of making and communicating health care decisions for him/herself
-Diagnosed with a terminal illness that will lead to death within six months. It is up to the attending physician to determine whether these criteria have been met.
-Must come to Oregon for the minimum 15-day process and take the life-ending prescription to die in Oregon (no residency requirement)
